Possibly the least disturbing entry on this list, season 1 episode 'Red Hood' has made its way onto this list by merit of one particularly unforgettable moment.

While the scenes depicting the ill-fated exploits of the Red Hood gang are much lighter than your standard Gotham fare, the Wayne Manor scenes are quietly unsettling.

After receiving a visit from old army colleague Reggie, Alfred begins to suspect that his former comrade might have a sinister motive. The scenes between Reggie and Bruce are noticeably uncomfortable, with the former soldier's attempts to teach Bruce crossing the line between well-intentioned and plain creepy.

But the true heart of this episode's disturbing nature lies in the Dollmaker's prison. When Fish Mooney finally gets to see the upper levels of the facility, she comes to understand that the people imprisoned below are there as unwilling organ donors.

During a conversation with the Dollmaker, Fish learns that he intends to harvest her eyes, so she grabs a nearby spoon and scoops one out herself. It's one of Gotham's most outrageous moments, and it makes for particularly uncomfortable viewing, but it does serve to highlight just how far Fish is willing to go to survive.
