Agnes is in the La Sirena sickbay nursing an upset tummy after eating two slices of red velvet cake the size of a Borg cube.

This is the first time we can see that the La Sirena's sickbay (which first appeared in "Stardust City Rag") is situated directly behind the mess area, which itself is located below the cockpit. The sound effects in sickbay consist of persistent beeping that are the same sounds from the Enterprise-D's sickbay and (for some reason) the transporter room aboard the USS Discovery in Star Trek: Discovery.

Desperate for the Romulans to stop tracking her, Jurati uses a replicator (actually a commercially available 3D printer made by Monoprice) to create a hypospray containing noranium hydride and injects herself with it. This apparently neutralizes the delicious-looking tracking device, but the EMH returns (after Agnes deactivated him in "Stardust City Rag") just in time for Jurati to foam at the mouth and slip into a coma.
