In the realm of Supernatural, Eve is not your average First Woman with a sinful past. A mysterious and powerful being known by the title Mother Of All, she was in the depths of Purgatory. Released by dragons onto Earth Eve uses a classy vessel: the body of a virgin girl, parking her ancient soul in an innocent body. Eve has the dangerous power to create.

What makes Eve so scary is that her true appearance is unknown. However, a glimpse of it through the lens of a video camera reveals a grey, withered, somewhat shadowed figure with unidentified features. Her eyes glow in a fiery yellow-orange, very much like hellfire itself, but they also appear opaque and milky-white in another scene. Dark veins run all over her body, and she bleeds watery black blood, just like her creations, the Leviathans.

After possessing a virgin girl, she later briefly takes on the form of Mary Winchester. And once Crowley performs an autopsy on her dead body he finds that the interiors of her vessels are altered with the addition of an egg sac containing hundreds or even thousands of eggs. Her brain apparently maintained the empathetic link with her creations (or children) even after her demise. Her brain has other abnormalities that come right out of a lovecraftian short story.

While Eve is not necessarily scary on the outside - at least not while she wanders the earth - the scary part is what hides underneath that human mask. Mimicking our behaviour and trying to seem human to an extend is super uncanny, if you ask me.
