The first throw from the top of the cage was planned, and sharp-eyed viewers can see Foley somewhat able to guide his descent through the Spanish announce table. When Foley stormed the top of the cage a second time, Undertaker chokeslammed him onto a chain-link partition, which came undone, sending Foley crashing to the mat in horrific fashion.

Terry Funk would later say that the prop guy intentionally rigged the roof in order to allow it the capacity to sag (for dramatic effect), but it wasn't meant to fully break loose.

The Undertaker, for his part, thought that Foley had legitimately been killed in the fall, as the roof breaking came as a surprise to him. Funk concurred, adding that when he hit the ring to check on his friend, Foley's eyes, "...weren't rolled back in his head, but they looked totally glazed over, like a dead fish's eyes." Foley was knocked unconscious from the impact, but thankfully managed to come around minutes later.
