There have been a handful of title changes at non-televised WWE and NXT live events in the past, and that always results in a lot of headlines. Making it a regular occurrence would be a mistake because championships in WWE have already been devalued in recent years, but it certainly wouldn't hurt to have it happen a little more frequently.
Wrestling fans these days are smarter than ever before, so they know that when they're watching a title match at a house show, the chances of it changing hands is somewhere between slim and none.
Putting some doubt into the minds of fans would be no bad thing, though, and if WWE were to start advertising the fact that a title is going to be on the line at one of these shows, people will buy tickets and those who can't attend will be glued to social media waiting to find out what happens. It will take time for that to happen, but with solid creaive decisions and some genuine surprises, the perception of these shows could rapidly change.
