The most obvious cause of death here is death by asphyxiation. Any situation in which you are expecting to breath more butt than oxygen is going to end badly if it goes on for too long.

Although exact figures of "death by facesitting" are tricky to get hold of, we do know that the average person will lose consciousness after around 40 seconds of smothering and will likely die in the next couple of minutes when oxygen concentrations dipping below 5%.
