1. Being Immortally Damned To Dormammu’s Torture
When Doctor Strange faced off against Dormammu in his film, he managed to get the immortal godlike being to back off after trapping him in a time loop. That was no picnic for either person involved, but when it was all said and done, Dormammu brought the pain on someone else.
When he was forced to return to the Dark Dimension, he took along some playthings, which was part of the deal he struck with Strange. Kaecilius and his zealots were pulled into the Dark Dimension, where they were to be subjected to the imaginative delights of Dormammu's torture.
As Kaecilius was being drawn into the Dark Dimension, Strange promised him that he would receive the eternal life he was promised in the Book of Cogliostro, but he wouldn't enjoy it. That's the thing about the Dark Dimension and Dormammu's insane wealth of power, the zealots could live indefinitely without the benefit of dying of old age.
Ultimately, being tortured for all time is probably one of those fates worse than death most people wouldn't wish on their greatest enemies. Screw up someone's taxes or have them stub their toes, but live forever in torturous pain sounds like a living Hell, which is exactly what it is.
