In this particular issue, the villain in question is a monocle-wearing Count. The upper-class cretin makes the mistake of throwing a sword at the Dark Knight, whose only inconvenience is that he was ticked off at the guy's audacity. He calls the fellow a "rat" and subsequently serves him an incredibly needless death.

Batman shuts the door, walks towards the man, gets on the other side of him and hits him with enough force to send him flying on to the very sword he had just thrown. He ends up with the weapon impaled through his neck and, of course, perishes. Batman did this when the man was unarmed, when he was no longer in any danger, and he had to orchestrate the situation by closing the door and positioning himself suitably for it to occur.

It's an outright murder in the true sense of the word.
