Feige has said that his original intention wasn't to set up a cinematic universe, but to simply make a good Iron Man film.
Apparently, the post-credits scene that started it all was haphazardly thrown together at the last minute. It was NEVER part of the plan.
When Samuel L. Jackson heard that an Iron Man film was coming, he called to see if he could get a role. Feige cast Jackson as Fury, had Brian Michael Bendis write the scene, and then filmed it the next day.
