Escaping from the loony bin, where his tendency to laugh when nervous earned him the nickname Dr Giggles, Dr Evan Rendell returns to the small town where his equally gonzo father was murdered and proceeds to take revenge on the townsfolk.

Minor characters are given lethal injections, impaled or torn apart, usually with a “witty” remark from the doc (this is a post-Nightmare On Elm Street slasher, after all). While using a stomach pump on one victim, he says, “I know. It sucks.” Before taking a golf club to another, he quips, “Time to do what doctors do best.”

So infectious is the doc’s humour that at the climax, the heroine gets in on the action, stabbing him twice and telling him, “Take two of these and call me in the morning!”
