Since first working together back in 2002, Leonardo DiCaprio has collaborated with Martin Scorsese for six cinematic efforts, the fourth being the immense and arguably underrated Shutter Island. The film was intense, nerve-wracking, and featured one of the all-time character twists with DiCaprio's Andrew Laeddis.

The twist, of course, was that DiCaprio was truly Laeddis, after spending much of the story acting as US Marshall Teddy Daniels on the hunt for the aforementioned Andrew. Turns out Daniels was a character composed by Laeddis' psyche to deal with the horrors of his past, in which he killed his own wife after finding she had murdered their children.

Dr. Cawley (Ben Kingsley) and Dr. Sheehan (Mark Ruffalo) allowed Andrew to play out this fantasy of his on the island in a radical attempt to prove to him that the story didn't make sense, and to finally reach him with the message of his true reality. If it didn't work, then it would be a lobotomy.

Though this was the last resort for Dr. Cawley, it was ultimately what came to pass, as Laeddis showed that he still believed he was Teddy Daniels. However, the fact that he had previously seemed to truly grasp the truth of his life suggests that he knew exactly who he was and that he would rather volunteer for a lobotomy through keeping his Teddy Daniels act up than live with the memory of his own past.
