Looking closely at the actor's top, a hole is visible in the centre of his chest, hinting at the return of the arc reactor, which was embedded there between in Iron Man 1.
This device was installed by Stark to prevent shrapnel that was lodged in his body from entering his heart and killing him - it also turned out to be an effective power source for the Iron Man armour.
Tony removed the gizmo in Iron Man 3 and has never reinstalled it since.
