Why He Has To Die
Chris Evans is another of the big name actors at Marvel with a rapidly ending contract, and while he has spoken about the possibility of extending it, it would still make sense for Captain America to follow his comic book twin and die at the end of a major story arc.
He might have survived Civil War, but Cap is now a symbol of heroism on a whole other level after going underground, and his martyrdom would be the smartest way to turn Tony Stark back onto the idea of the Avengers Initiative. Again, it might not last (as it didn't in the comics) but having him disappear for a few years and letting Bucky or Falcon step in as the new Cap would make sense.
When It'll Happen
In Infinity War, as Thanos is rising to power.
How He'll Die
It would be unfortunate for him to be one of the victims when Thanos clicks his fingers, because Cap deserves more, but a character as big as him die in such circumstances would definitely be a statement of Thanos' power and threat.
