5. Iosefka's Clinic - Bloodborne
The world of Bloodborne isn't really made for happy endings, but when the game offers you an opportunity to play the hero and save some people, it could at least have the courtesy to deliver on that promise.
Iosefka's Clinic is a location in Bloodborne where the player can send survivors scattered throughout Yharnam. The clinic's creator, Iosefka, is a compassionate and caring woman, which makes the whole place appear extra safe and legitimate. However, when the player returns to Iosefka after they defeat Father Gascoigne, her personality will undergo a drastic change. The woman will start acting suspicious, becoming very insistent on sending more survivors her way.
It turns out that while you were gone, Iosefka was replaced by an imposter, and every single person you sent to her was tortured and experimented on.
The worst part about this is that even if you did manage to pick up on the healer's shift in personality early on, you can't save the people you sent to Iosefka when she was still herself. The game tricks you into thinking you can help people find a safe refuge, and when you realize this isn't the case, it's already too late.
