4. Grand Theft Auto IV - That Can't Be Practical

The original cover for Grand Theft Auto IV had two mistakes, and you can find both of them in the same panel of the graphic novel-like box art.

In the panel we see protagonist Niko speeding towards the camera in a high speed chase with the police. At first, you'll notice that the license plate on his car is reversed. No biggie - on the final box, it was covered by the ESRB rating. Presumably, they mirrored an image for better composition and hid the part that didn't work under a logo. That's actually pretty smart design.

However, the same can't be said for the other error. Take a close look at Niko and you'll see that his head is in front of his side-view mirror.

To be fair, such a driving position would be totally possible. But it couldn't possibly be comfortable. The back strain alone seems like it would be an unnecessary distraction during a police pursuit. Fortunately, future prints of the game corrected both mistakes.
