Agent 47, the Hitman series' protagonist, is a renown master of stealth. Well, on paper, depending on who's playing. As such, each mission should be executed just like only the target(s) should, with no unwarranted casualties.

Unless there's something in it for the player.

Blood Money, the fourth series entry, had achievements added for its Xbox 360 launch. Higher difficulty challenges, collecting all weapons... and some creative flair.

One such achievement (and later trophy for the HD collection) is the aptly named 47 kills. A play on the agent's name, the requirement for this had players kill no more or less than forty seven people in a mission.

At most, a mission will have three/four targets. That means, unfortunately, that those crowd-populating NPC's have to make up the numbers.

It doesn't matter how the player does it, just that they remember to keep track of the amount. In can be indiscriminate, completely random passers by that add to the tally.

As long as the main targets are in there, and the mission is finished, that's all you need. Just don't overdo it.
