2. The Boy - The Last Guardian
Of course, the protagonist of The Last Guardian doesn't actually die story-wise, but that doesn't mean the various Game Over conditions weren't thoroughly unpleasant.
Firstly, if the player fails to fend off the armoured knights, the boy will be taken through a bright white door and presumably gifted to the game's final boss, The Master of the Valley, who seemingly sustains his immortality by absorbing the life force of children.
If that's not unsettling enough, you can also give the boy some gnarly deaths while platforming with Trico. The finicky AI means that he won't always obey your every command, and so he may shift out of the way just as you leap to land on his back, or may fail to rescue you from a long drop because he's busy staring somewhere else.
Considering the Boy can actually survive some impressive falls mostly unscathed (apart from a sprained ankle that miraculously fixes itself after 20 seconds), seeing him ignored by Trico and realistically smash into some rocks on his deathly fall to the ground is pretty damn disturbing.
