In today's age of cloud storage and Google Calendar, the idea of using your flesh as a journal seems a little bit old-hat, but that's pretty much what life was like for everyone pre-2007, including Planescape: Torment's hulking hero. Desperate to piece together the events of his past lives, the Nameless One (TNO) marks himself with scratches and tattoos to keep track of his mind-bending inter-dimensional existence. His back is covered with symbols reminding TNO of his past lives. On his right shoulder is the Symbol of Torment - a permanent reminder of all the people our troubled hero let down, stepped on, and betrayed along his journey. It depicts Deionarra, the spirit who loved a previous incarnation of the Nameless One, whose fate is implied to be intertwined with TNO's. But TNO's markings serve a higher purpose. As you progress through the game, you accrue more tattoos for TNO, endowing him with special abilities, powers, and stat-boosters, possibly inspiring a certain backpacker to ink himself up in a similarly empowering way some years later...
