It just wouldn’t be a list about censorship without a Rockstar game making the top spot. The Manhunt series is infamous for making GTA look like Lee Carvallo’s Putting Challenge.
The 2007 sequel to Manhunt’s original release was so extreme it received an AO (Adults Only) rating, a rating pretty much exclusively meant for pornographic games, which meant it could not be sold on any console. This effectively banned it from being sold full stop.
Rockstar had to censor the game, removing all but two decapitations, as well a particularly gruesome execution involving removing the victim’s testicles with a pair of pliers. A blurring effect was also added to the screen during executions.
All this was enough to secure an Mature rating rather than an AO rating, however the Wii still refused to offer the game, and it went through yet another round of censoring in order to be released on the console.
