2. Higgs
For most of Death Stranding Sam isn't actually being pursued by Clifford all that often and he isn't aware of the real Extinction Entity - he is actually being tormented by Higgs, a vessel for Amelie and leader of the Homo Demons. A terrorist group which only look to seek havoc in an already broken world.
He is yet another DOOMS, but with a difference, as he gained his abilities from killing his uncle and witnessing the transformation to BT. Even though he is an extremely ruthless individual, Higgs is in fact, very calculated and intelligent which makes him a very formidable foe.
It isn't till his meeting with Amelie that he becomes hellbent on ensuring the sixth extinction event though and during this time is when she bestows complex Chiralium powers upon him. This means he can control advanced forms of BTs, Teleportation, Clairvoyance Senses, Perceived Immortality and even summon Timefall.
Higgs also has an affiliation for Egyptian mythology as he can be seen covering his clothing with the iconography of said culture. This plays into his god complex and the golden mask is the cherry on top.
After Higgs is defeated it is left open to if he really is dead or not, but one way or another if there is a second Death stranding, we won't have seen the last of him.
