In previous gameplay trailers it was always clear that Franklin was purchasing some illegal narcotics but it remained unclear if it was just a cinematic within the game. This passage clarifies everything essentially saying that drugs function like alcohol in past games. You can indulge in them all you want and create your own adaptation of Requiem For A Dream, but it comes with temporary gameplay kickbacks. The effects will assuredly be a hoot providing some major laughs though.
