This title is an assumption, as in the trailer Snake is shown covering his face in someone's ashes, although it could be anyone from a string of fallen soldiers to The Boss herself from MGS 3.
Either way the concept is pretty morbid, and potentially shows the first instance of Big Boss' descent into madness.
