Speaking of a descent into madness, look to this shot being somewhere near the close of the game, assumedly when Big Boss has lost everyone and is left, a smouldering wreck of his life behind, contemplating exactly how to move forward.
It's a truly horrifying image, and one that if Kojima can balance the tone of things like this with the randomness of the cardboard boxes and scantily-clad super-heroes, could make for a very emotionally-engaging story.
