Both Snake and his right hand man Kaz look to be going through hell at the start of Phantom Pain, being they were both involved in the helicopter crash that causes Snake's coma, and following that Kaz himself is captured by enemy forces.

It looks like one of the earliest missions is retrieving him, and although he makes it back, it's clear from this shot that Kaz barely makes it out alive.
