He's been tormented at the hands of the truly evil Ramsey Snow, and has been reduced to little more than a household pet for the Bolton family, who are set to take Winterfell and rule the North. After the brutality suffered by Reek, the tension is palpable during a simple bath, which shows off the dehumanisation of Theon, and his body laced with scars. It's grim, but it's a reminder of how his character has been warped.
