The rich, known as Meths, use their wealth to buy new bodies, or 'sleeves', effectively living forever, while the poor must make do with what they have and can buy. The skilled assassin uses the element of surprise to tear a hole in Ortega's shoulder and abdomen, using his martial arts skills to keep the two at bay.

One only has to look at the bloody carnage left in the elevator to realise how ruthlessly ruinous the weapon is. While Ortega survives the attack, she barely makes it to a hospital, and the extensive damage to her shoulder causes her to lose her arm.
