Basically this entire episode is a twist, with viewers constantly having their expectations subverted and finding themselves asking one question when the credits roll. What, if any of Janeway and Kashyk's relationship was real?
The first twist comes when Kashyk arrives in a private ship claiming he no longer wants to be a Devore inspector. This development is such a shock, because Kashyk showed no desire to leave his position in earlier encounters with Voyager. Janeway is initially wary, but as they begin to work together, a mutual attraction begins to develop.
Soon Kashyk leaves the ship under the guise of saving the Voyager crew time to get the telepaths they're transporting to the wormhole they've been seeking. Once back on board with the inspecting crew though, Kashyk reveals himself as a counter-agent and all hope seems to be lost. It seems he has managed to trick Janeway into believing he wants to help the telepaths.
Janeway isn't having it though, and in the final twist of the episode she reveals the telepaths were no longer on the ship and she had never trusted Kashyk. It's this development that has viewers cheering and also in utter disbelief that such a manipulation between the two of them had occurred with neither one trusting the other. Just when it seems Janeway has been bested, she proves how intelligent she really is.
Its shock after shock in this episode, making it one of Voyager's best.
