While hunting a Djinn, Dean is attacked in an abandoned warehouse and taken to an alternate reality in which his mother hasn't died, and the brothers lead normal lives. The whole premise of Supernatural is that everything happened because Dean and his dad couldn't save his mum from the demon Azazel, so theoretically this should have made all his dreams come true

The episode shows why getting his wish to have her back would have negative consequences. Sam is now successfully studying to become a lawyer and engaged to Jessica, and Dean is in a relationship with Carmen, but the brothers are no longer close as they never had the chance to develop their relationship as they never became hunters together.

Dean also starts to realise that all the people they saved in the series are now dead, which causes him incredible guilt. In a further sad twist, their father also died of a stroke. When Dean reaches out to Sam to help, they discover that the Djinn is keeping Dean in a hallucinatory state, so he is compliant while it feeds on him. Dean escapes by killing himself in the dream, despite his mother, girlfriend and Jessica trying to stop him.
