Throughout Series 6, The Doctor has been hounded by the knowledge that he WILL die at Lake Silencio at the hands of The Silence. Since it's a fixed point in time, he can't change it, so he does the most human thing he's probably ever done: he runs. But of course, one can only run for so long. So The Doctor goes to face his destiny.

Only for the assassin, in this case River, to say no to shooting him. And since - again - this is a fixed point in time, we get to see what happens when that particular rule is broken. Namely, everything goes bloody crazy. Now, we've seen what happens when fixed points in time are broken before, like with Rose's Dad. But since The Doctor is a time traveler, the consequences are a bit more...shall we say, dire.

All of time unravels and tries to put itself back together, making for a fascinatingly bonkers world that The Doctor has to fix. It's a hell of a finale to one of the best seasons in this entire era of Who.
