In season six, Alexandria was overrun with walkers (despite Rick and co.'s best efforts), and the survivors trapped in the community had to mount a daring escape. Covering themselves in walker guts, Rick, Carl, Gabriel and the remaining Anderson family - Jessie, Ron and Sam - moved into the horde in hopes of getting to safety.

Unfortunately, no one seemed to account for the fact that little Sam was the most annoying and naive character of all time. Out in the horde, Sam started begging his mother to turn back, grabbing the attention of the walkers and causing Sam and Jessie to be devoured.

This was pretty dark, but things got worse in a hurry when Ron went to kill Rick in a rage and, as Michonne killed him at the last second, he let off a shot that hit Carl in the face, leading to Carl losing an eye. The whole episode is incredibly dark, but this sequence easily takes the cake as its most unsettling and cruel moment.
