The already bleak situation for Donna and the country as a whole gets progressively worse when any hope of American aid is halted by the birth of the Adipose resulting in the deaths of millions of people. Except that the deaths caused by the Adipose birthing plan only happened because of the Doctor and Donna intervening. While you could easily rationalise this in some way like the American arm of UNIT getting involved or the Eighth Doctor's companion Grace Holloway investigating, the point remains that its an inconsistency for the sake of moving the plot forward. As is the lessening of the damage caused by the Titanic despite Voyage Of The Damned, Partners In Crime, and Turn Left all being written by Russell T. Davies.
