2. Name Of The Doctor
In 'The Name Of The Doctor', we find The Doctor's grave. Like the grave of any time traveller, this is a scar in space-time, showing the effect the Doctor has had on the universe. Clara travels inside and is able to see every Doctor up until then, so why weren't any of the previous incarnations of The Doctor available to see within that timeline?
The thing to remember is, this is not just The Doctor's timeline. If it were, then Clara would have interacted with The War Doctor several times, rather than just finding him buried at the bottom, and it's finding him hidden away that gives us the clue we need to understand this scene.
This is The Doctor's timeline as tempered by his own dying mind. Just as his shame at his own actions caused him to repress The War Doctor, so that version of himself is buried at the bottom where it can barely be found.
While that is conscious repression, all of the Doctor's experiences working for The Division have been totally removed from his mind by this point. They literally aren't there to grow into the time scar that marks his grave.
