3. Tuvix (Tuvix)
Tuvix is, in many ways, Captain Janeway's Kobayashi Maru test. There was simply no way to win here. So while this absolutely ranks on any list of the worst things that she has ever done, it is of course also done in the act of saving lives as well.
In this writer's mind, there is little doubt that Janeway made the right call. That doesn't mean it was a good call though. Tuvix, though short-lived, bore all of the memories of both Tuvok and Neelix, creating this new form of life. To erase him was to execute him, awful as the decision may be. Even the Doctor, programmed though he was to follow order, could not assist in the procedure.
Janeway took the full responsibilty here and it does truly rank as one of the worst things she has ever done. Was it a negative action though? This falls squarely into the realm of necessary evil. Tuvok and Neelix needed to be saved, which had to come at the cost of Tuvix's life.
As revealed in her autobiography, ghost written by Dr. Una McCormack, this was a choice that Janeway never, ever forgot.
