7. Carlito
During Carlito's final few years in WWE, reports would often hit the dirt sheets about him not taking his job seriously and showing a lack of desire to move up the card. Along the way, it seems like he generated a lot of backstage heat and that seemingly made his Wellness Policy violation in May 2010 the final straw for his employers.
After learning that he had become addicted to painkillers, WWE requested that Carlito seek help and go to rehab before being allowed to return to television. He flat out refused and was released from the company as a result. This was a rare case of WWE firing someone for a first offence but he clearly didn't want help at the time and Vince McMahon likely didn't want another high-profile death on his hands.
Thankfully, Carlito would clean himself up in the years that followed and has since been seen mostly on the independent scene. He's also made sporadic appearances for WWE (such as at the 2014 Hall of Fame ceremony alongside his family) and seems to be on good terms with them. It's still hard not to wonder what might have been for his career had he accepted help, though.
