Rey Mysterio is one of the most beloved WWE Superstars of all-time but even he's guilty of making some mistakes during his run with the sports entertainment giant. Way back in 2009, the Master of the 619 found himself benched for his first violation of the Wellness Policy, but was quick to later point out that it was due to painkillers which had been legally prescribed to him. Unfortunately, being on vacation/a promotional tour meant he was unable to provide the proof needed to clear his name in time.

Then, nearly three years to the day, Mysterio failed another random drug test, this time for amphetamines. He argued that he hadn't used any medications in a way which should result in his suspension but as he was at home nursing an injury at the time, he once again failed to provide WWE with the necessary proof. This happening when it did wasn't all bad, though, because he wasn't suddenly pulled from television like some of the other Superstars listed here; it just delayed his return by a couple of months.

Unfortunately, Mysterio never really rose back up the card after that and would later leave the company in 2015 before making his widely celebrated return in 2018. Since then, he's has made a splendid return to form, but it's unknown whether a third strike would have serious consequences or if he started afresh after signing a new contract.
