Ruby is introduced in issue #33 of The Defenders, as the soon-to-be newest member of The Headmen (or Headpeople). This group of villains believe that the world should be ruled by only those most intelligent people, like themselves (naturally). It does appear as if Ruby took the head part of that name rather literally.

Her outfit consists of a popped collar leotard, gloves, boots and most importantly what looks like a round red ball where her head would be. In actuality, her head is an organic computer that she made out of a malleable plastic that can reform itself into anything she wants.

As a brilliant scientist she could have had this technology attached to her as a suit (or if she was really set on cutting off a limb she could have chosen some less important part of her body than her head). Instead, her choice is incredibly impractical. As one of the other members of The Headmen points out, "We're all outcasts, but at least our heads are human!"
