Because Al Simmons was a world-class assassin, he was condemned to hell when he died, where he was reborn as a hellspawn. Despite being a minion of hell, he intended to vanquish all evil, hoping this would allow him to reclaim his humanity.

After Spawn averted The Rapture, God reverted him to his human form so he could reunite with his wife, Wanda. As he approached his old home, Al heard Wanda having an argument with her current husband, Terry. During the quarrel, Terry complained that he felt like Wanda loved Al more than him and he could never compete with him.

In that moment, Wanda revealed a dark truth about her ex-husband. Wanda got so tired of Al putting his work ahead of her, she intentionally stopped taking birth-control pills, causing her to fall pregnant. Wanda assumed Al would give her more attention if he was carrying her child. When Al learned this, he punched her in the stomach, killing the baby.

In that moment, Al realised that he was the monster that everyone said he was. Concluding that Wanda was too good for her, Al left her and returned to life as a hellspawn.
