Albert Simmons was a highly decorated Marine, Secret Service, and CIA Agent, and member of a multiple agency task force under the direct command of Jason Wynn. During a mission in Botswana, Wynn hired Simmons' partner Chapel to kill him. Sent to Hell, Al made a deal to become a Hellspawn for the opportunity to see his wife Wanda one last time. The new Hellspawn was returned to Earth with few memories, a horribly disfigured body, and a disgusting demonic guardian named the Violator.

Being referred to as Spawn, Simmons realized that it was five years later, that his wife had married his best friend and they now had a daughter. He took up residency on the streets and began to fight for the oppressed there. Spawn fought against mobsters and criminals and killed a paedophile named Billy Kincaid.

Spawn had found through his battle that his powers had grown. His costume transformed, and was, in fact, a living organism. When it first transformed after his battle with the Redeemer, the cape and chain shape shifted to protect him when attacked, and the suit itself would feed off of souls. Simmons now wears a living symbiotic suit, Leetha of the 7th House of K, which is attached to his central nervous system and will protect him even when he is unconscious.
