When Scarlet Witch went insane, she rewrote all of reality. In this new world, mutants were the dominant species over humans and Magneto was their emperor.
The Peter Parker of this world enjoyed the life his normal self had always dreamed of. Spider-Man was the world's favourite mutant superhero, actor, pro-wrestler and businessman and enjoyed a life of luxury surrounded by his wife Gwen, their son Richie and all their not-dead loved ones.
However, that life came crashing down when the Green Goblin appeared. The Goblin attacked Peter's family and revealed to the world Spider-Man's greatest secret, he wasn't really a mutant.
After causing havoc, the Green Goblin was apprehended by Gwen and Peter's bodyguard Rhino. Gwen unmasked the Goblin only to reveal it was actually Peter himself.
It turned out that Peter had been plagued with nightmares of how the world had been before and the deaths of Gwen and Uncle Ben. The stress of these dreams caused him to snap and subconsciously create the identity of the Green Goblin.
In an even darker What If? ending, Gwen and Richie survived the eventual transition back to the normal Marvel Universe. The stress of having Gwen and Mary Jane around caused Peter to become the Green Goblin again. He tried to kill MJ, but regained his senses at the last minute and threw himself in front of his glider to save her.
