There are some things that are worse than any bodily injury or personal tragedy you could conceive of. While the list is very small, it contains one definite entry, written in all capitals, and it's the phrase "TURNING INTO A LITERAL SPIDER CREATURE".
Which the ever-unfortunate Peter Parker has the blessing of doing during the wonderfully named 'Spider Island' arc, which sees its villain bite Spider-Man and inject him with a toxin that changes his body into a nightmarish spider monster, in what appears to be a very painful transformation.
This leaves Spider-Man in a twisted spider-body until he is mercifully killed, at which point he promptly births himself, because at this point, why not. Should Peter retain literally any memory of any of these events, it's definitely the top of the list of trauma.
