How Did He Die?

As with Marrow (and seemingly half the X-Men's enemies), Sabretooth has been an X-Man at certain points in the past, so his name finds itself on the list. In one of his many mano-y-mano battles with Wolverine, Sabretooth had been reduced to a feral state by the shadowy figure Romulus, and eventually broke through his blind rage to ask Wolverine to kill him.

Wolverine, who had recently come into possession of a sword forged from his own soul, gladly obliged him. Sabretooth was decapitated and left to freeze in the snow.

How Did He Come Back?

Initially Sabretooth came back with no reasoning at all, until it was revealed that Romulus had cloned Sabretooth a bunch of times, and the one that Wolverine beheaded was one of those replicants. A poor explanation, especially considering that Wolverine had come across Sabretooth's soul in hell at one point during Jason Aaron's run, which seems like a pretty clear indication that he was actually, properly dead.
