2. Not Finishing Off Anakin - Star Wars: Revenge Of The Sith

Oh Obi-Wan, you were so close.

At the end of Revenge of the Sith, Obi-Wan and an Anakin now fully in the grip of the dark side of the Force engage in a fierce lightsaber duel which is arguably the best of the entire prequel trilogy.

By the end of the duel on the volcanic planet of Mustafar, Obi-Wan has gained the high ground on the bank of a lava river while Anakin stands on a platform below him. Despite Obi-Wan warning him that the fight was done, Anakin's pride and anger blind him and he leaps at his former Master - with Kenobi finishing the duel by cutting Anakin into various parts.

Understandably, Obi-Wan is upset and uses this moment to vent his anger and sadness at Anakin, reminding him that he was the Chosen One, the One who should have brought balance to the Force, before turning his back on his Padawan and letting the lava and heat finish him off.

Or so he thought.

All Obi-Wan had to do was make one more swipe with his lightsaber. Jedi were no strangers to beheading people, after all, and him doing so would have prevented the further rise of Darth Vader, saved millions of lives, and probably ended the Galactic Civil War long before the Ewoks had to get involved.
