Psychiatrist, Isaac Barr (Richard Gere) is treating Uma Thurman's Diana Baylor for obsessive-compulsive disorder. Her sister, Heather Evans (Kim Basinger) tries to shed more light on their troubled family history, resulting in Isaac and Heather falling in love. However, Heather also has troubles; she is unhappily married to a gangster (Eric Roberts) and suffers from pathological intoxication, both of which threaten to tear her and Isaac apart.

An homage to Hitchcock's Vertigo, Final Analysis is also set in San Francisco, providing a sultry, elegant look to the city, which is hard to resist. It also blends the film noir aspects of the plot with thriller and horror aspects to make this a truly unforgettable cinematic experience.

Eric Roberts, Keith David, Paul Guilfoyle, and Harris Yulin receive memorable supporting roles, whilst George Fenton provides a beautiful musical score for the film.

It was the second pairing of Richard Gere and Kim Basinger, their first being 1986's No Mercy.
