This mini reactor is the thing keeping him alive since surgery to remove it might kill him.
Not the case, according to Iron Man 3. Nope, all he needs is the help of a really good surgeon and presto, no more shrapnel! This implies that if he was bothered he probably could have gotten it removed at any point after his return from captivity, so why he’d chose to walk around with life-threatening shrapnel in his chest for a few movies is a head scratcher.
This magic surgery is yet another issue MCU fanatics have with Iron Man 3, and is a deus ex machina they call BS on.
