4. Alex Dies Off-Screen - Final Destination 2
Even though the original Final Destination concluded on something of a bleak, open-ended note, protagonists Alex (Devon Sawa) and Clear (Ali Larter) were still very much alive when the end credits rolled - albeit having just witnessed the death of their friend Carter (Kerr Smith).
However, for reasons which haven't ever really been made clear, Devon Sawa didn't return for the second movie, and so the filmmakers made the rather drastic decision to reveal that he'd been killed between films by a falling brick of all things.
Though the smart call here would've clearly been to simply have Alex go missing in action, in turn leaving the door open for Sawa to return in a future film, Clear instead states that she was a witness to the incident and even shows a photo of Alex's corpse to new protagonist Kimberly (A. J. Cook).
That's... about as definitive as things get, and if that's not bad enough, Clear herself ends up getting blown to smithereens before the end of the movie.
