2. Gatekeeper - Skyrim
Of all the games out in practically the entire world, the one you'd expect to be most completed by its general fanbase is Skyrim. It's massive, so you don't expect every achievement to have been loyally done, but players tend to invest big hours in it, which you'd presume would translate into most "basic" achievements being nabbed by most.
And yet, only sixty-six percent of players have the suitably named Gatekeeper award, which you get for joining the Winterhold College - who, for anyone who hasn't played it, is basically just the Mages' Guild but named different.
There are a fair few reasons that explain why this is the case. Firstly, Winterhold is at the very top right of the map, meaning that it can easily be one of the last places you ever journey to. To make matters a little more complex, it's also an area that doesn't have a huge wealth of quests around it, meaning you can't accidentally rock up there for many other reasons than curiosity.
But it's still insane, because the "test" you do to get in literally requires you to just know magic, do a dragonborn shout, or pay thirty gold - and most players should be able to provide one of these things from practically the start of the game.
