6. Pathologic 2
On its surface, Pathologic 2's horror stems from disease.
In this game, you take control of a surgeon returning to a town about to be ravished by the plague. As the days pass, the disease starts to take more lives and spread across the town, and it's up to you to try and figure out a way to stop it. Of course, it's not as simple as finding a cure, as you have to play politics, convince the people with power and resources it's in their best interests to help you, all the while attempting to stop any violence in the streets as people get more and more desperate.
There's of course another layer to this though, as the town itself (alongside its inhabitants) isn't what you'd call "normal". It's full of creepy figures living out their own stories, as well as fourth-wall breaking beings who comment on the player's progress through the title.
It's a game essentially impossible to sum up in a list like this, but if you wanna play a complex horror RPG that absolutely hates your guts, you'll get none meaner than Pathologic 2.
