First and foremost, it drastically tones down the colours and makes everything look darker and grittier; as a result, the environment appears far more hostile and formidable. Perhaps due to the harsher lighting effects, the mod also makes fog look thicker and more impenetrable, creating this Silent Hill like atmosphere which is bound to send shivers up your spine.
