It shouldn’t come as a great surprise that Bloodborne is a brutal game with a brutal game world that offers up brutal outcomes for any choice you make, including the ethical ones, but just in case it does, consider yourself warned.

If you try to save all of the friendly NPCs you meet out and about in the nasty game world, you’ll get them all killed. It doesn’t matter if you send them all to Ioesefka’s Clinic or to the Chapel, you’ll more than likely lead most of them to their untimely deaths. The caveat to this is if you send the Suspicious Beggar from the Forbidden Woods to Ioesefka’s Clinic.

Do this and you’ll still get the guy killed but since he’s a cannibal you won’t let him loose on the rest of your townies. Send anybody to the clinic and Ioesefka will experiment on them and kill them horribly, send the Beggar to the Chapel and he’ll systematically kill off everybody there. Also horribly.

To be fair, though, when you meet the guy he is munching on a kid’s corpse, so, this one is sort of on you.
