This one might seem a little confusing for Dark Souls newbies, but essentially the player has leech-like worms on his body causing his bleed bar to fill up. With low health, he dashes to the bonfire up ahead to restore his vitality, but alas, the bleed bar reaches max before he can and causes him to die via blood loss, when he's just mere inches from the bonfire. 

Still, like most deaths in Dark Souls, it's the player's fault rather than a cheap mechanic: he could've used fire to get rid of the worms, but instead took a chance and paid the ultimate price. Lesson learned.
